Gather the data for the MDS Section GG0130 and GG0170 (Self-care and Mobility) using this 31-day form.  Can be used for a 5-day PPS (or for States that need Section GG for OBRA).  Best printed as back-to-back, making the front pages for data collection and the back pages for signatures and extra notes.  The first 4 pages are for Self-Care and the next 6 pages are for Mobility items.  Newly added last page has images associated with the codes for visual aid.  A total of 11 pages in color.  Can be used for clinician or CNA documentation.
